Please read the following statements

The Member declares that they have:

1. Never been declared bankrupt and have never been disqualified from acting as a company director.
2. Never had any County Court Judgments (CCJs) or Sheriff Court Decrees issued against them.
3. Never been an officer of a company that has been declared insolvent, had a receiver or liquidator appointed, or entered into any arrangement with creditors under the Insolvency Act 1986.
4. Never had a proposal for similar insurance (made by or on behalf of the Member, their business, or any predecessor entity) cancelled, refused renewal, avoided by insurers, or subjected to special terms (other than general market-wide increases).
5. Never been convicted, nor have any prosecutions pending, nor been issued a formal police caution for any criminal offence other than motoring offences.
6. Never been subject to any prosecution, prohibition notice, or improvement order under Health & Safety legislation within the last five (5) years.
7. Never had an application for the renewal or transfer of a licence refused, and have no intention to apply for the transfer of a licence within the next twelve (12) months.
8. No knowledge of any fraud, dishonesty, bankruptcy or administration order.
9. Been domiciled in the United Kingdom only, and all past and expected future turnover is derived solely from clients within the United Kingdom.
10. No knowledge of any claims made against them.
